Coordinating care for a family member or friend with chronic conditions can be overwhelming and complex. We understand how difficult it can be to manage—especially when you are providing all the support they need on your own.

Circle Home Telehealth Services provides care coordination services to help ensure your loved one gets the services, treatments, and support they need to better understand and manage their conditions and avoid complications so they can live as independently as possible—at home. Having a dedicated nurse care coordinator (RN) can be a lifeline by offering guidance, coordinating medical care and services, and developing a comprehensive, individualized care plan to provide the support you both need.

Care coordination covers:

  • Answering questions about medications, treatments, visits, therapies
  • Talking through your concerns
  • Teaching and guidance on disease and symptom management, and preventing complications
  • Assisting in scheduling appointments and tests
  • Making sure prescriptions are ordered and filled
  • Reviewing and explaining medications
  • Connecting to needed services and resources
  • Helping to get equipment and supplies
  • Assessing need for other support services

Circle Home Telehealth Services offers a dedicated nurse care coordinator (RN) who will help your loved one get all the care they need that has been prescribed for them and will serve as an extension of their primary doctor or nurse practitioner as well as a bridge to specialists, pharmacy, and other providers. Our program helps eliminate barriers and assists you to:

  • Arrange transportation to appointments
  • Understand complex treatment plans or instructions
  • Get medications and help you know how and when they are taken
  • Have clear explanations about care treatment plans
  • Make the home safer
  • Avoid emergency room visits, hospital stays, and overcrowded waiting rooms
  • Be an advocate for your loved one so their choices about their care is known to all involved in their care

Insurance Coverage and Costs

Care coordination services provided through Circle Home Telehealth Services are covered by Medicare and, like other medical care, may be subject to an annual deductible and copays. If your loved one has supplemental insurance or a Medicare Advantage Plan, the copay is typically covered with little or no out-of-pocket expense. Prior to starting any services, we will find out whether there will be cost sharing and help you to better understand coverage, costs and how best to handle these.
